Thank you so much for visiting our website and we hope we will be able to help you with any non-medical questions or concerns that you might have after having your ICD fitted.
Any questions of a medical nature must be discussed with your GP or Cardiologist.
As we are a self-supporting voluntary group, the Membership Fee of £12.00 per year greatly helps towards the cost of producing the Monthly Newsletter, Annual Prize Draw, Telephone Help and Advice Lines, Social Gatherings and the continued development of this website etc.
You will receive a copy of our bi-monthly newsletter, by post or e-mail and also be given a membership card and number. Your membership number will be entered into a cash prize draw each December and drawn at our Annual Christmas Lunch.
For example, the total prize money paid out each year is £75.00.
1st Prize - £35.00
2nd Prize - £25.00
3rd Prize - £15.00
We also obtain the latest DVLA Driving Rules and Regulations information for our Members.
An added advantage of becoming a Member of the ICD Patient and Family Heart Support Group is that you will have access to ICD Friendly Holiday Insurance Companies.

Please now fill in your Membership Form and send this to us together with your fee of £12.00 in order to become a Member.

PLEASE NOTE:- As per the Data Protection Act 1998 Rules and Regulations, any information supplied either on the telephone or in writing to us must remain strictly confidential and under no circumstances will any information be passed on to anyone else without your prior approval.
Similarly, we also have our own rules and regulations on Patients Confidentiality, i.e. no information held by ourselves will be divulged to a third party, without your prior consent.
You can also be assured that should you decline to renew your membership at anytime, your personal details held on our database will be destroyed after 21 days.
